What is it?
The ordered execution record that lets a governed experience be inspected later.
The Operational Ledger records the important steps of a governed resolution so the experience can show how an answer was formed.
In Proof Space, the ledger is read-only. It explains the path behind the answer; it does not create a new organizational workflow.
Organizational Reality
A decision is made during a busy workday. Several sources are checked, a boundary is applied, and a limitation is noted.
Weeks later, someone asks what happened. Without an ordered record, the organization has fragments of memory rather than a reliable account.
Why it happens
