Provenance
What is it?
The trace from a source to the Assertion or answer that used it.
Provenance lets someone inspect where organizational understanding came from and how it moved through SALARA.
It starts at the Knowledge Source and remains important through evidence, Assertions, Replay, and governed answers.
Organizational Reality
A document says something important. It affects pay, access, leave, eligibility, or customer commitments.
Nobody knows who wrote it, when it changed, whether it is still valid, or which later answers have depended on it.
Why it happens
This commonly happens because organizations preserve content more reliably than custody. The statement survives, but the history that makes it trustworthy decays.
