A published Assertion can be selected in a governed answer when its scope, applicability, and effective time match the question.

Organizational Reality

The approved intelligence exists. Some employees still use older guidance.

A new answer may be correct in one channel while an outdated answer keeps circulating in another. The organization has approved understanding but has not made its active status clear enough.

Why it happens

This commonly happens because current understanding does not automatically replace old organizational memory. People keep using what they learned unless active guidance is made clear.
